By Joining this trip, I confirm that I am 18+ years old and wish to participate in adventure trips/activities organized by ASC. I confirm that I am in good physical health, and I am aware of the difficulty rating, and I am confident of my ability to take part in the said activity. I acknowledge that I am aware and understood of the safety rules in relation to the activity. I certify to the best of my knowledge that I do not have any medical condition which might make me more likely to sustain an injury. I confirm that I am not under the adverse influence of drinks or drugs. To the best of my knowledge, I am not pregnant. (Applicable for females only). I acknowledge that I am fully aware of the inherent risks associated with participation in the trip/activity and to the maximum extent allowed by law, I waive, release and discharge ASC, its organizers, member associates from any negligence, claims, losses cause of action including but not limited to death, personal injury or property damage arising out of my own voluntary participation in the above-mentioned activity. To reduce the risks, I agree to follow all the instructions given to me by ASC. In the event of an accident, or loss or damage to the personal effects, I acknowledge that ASC will not be liable for any direct or indirect loss damage or any injury arising from or in connection with the adventure activity and I waive all any claims against ASC in this respect. Event organizers or event hosts will not be held responsible for any accident, injury, or loss occurring because of any group event or in connection with travel to such event, however caused. Any medical conditions that may affect the participants during the activity should be notified to the trip organizer. I hereby give permissions for ASC and its organizers to secure medical treatment for me in case of any emergency. I authorize the physician or medical personnel or certified first-aider present to provide treatment deemed necessary to them. I will follow organizers instructions, and I will not deviate from the planned route. I will not litter, smoke, or drink alcohol. Rules and regulations have been adopted for the safe enjoyment of the activity and I agree to the regulations. ASC will not be held responsible for the consequences arise from any violation of the above-mentioned rules knowingly or unknowingly by the participants.
